Associate (Civilian / Spouse) Membership Application Form
1. You may apply for Associate (Civilian) Membership after retirement from the service as a Warrant Officer or Specialist. 
    Applicant must be in full-time service with SAF for at least 10 years.
2. Spouses of THE CHEVRONS Ordinary Members are eligible to apply the Associate (Spouse) Membership.

Terms & Conditions:
i.   Approval of application is at the sole discretion of the Management Committee of the Club.
ii.  Acceptance of application form or payment DOES NOT constitute to the approval of membership application.
iii. The Club is not obliged to give reasons of its rejection.

Membership Card Replacement/Re-issue
To replace a lost card or damaged membership card. Please note that there is an administration charge of $10.00 for membership card replacement.

Mode of Payment:
1. Make payment directly at our Reception, or
2. Send us a crossed cheque payable to “SAF Warrant Officers & Specialists Club”.

Please be reminded to:
i.   Indicate your name, NRIC & contact number behind your cheque
ii.  Indicate that the cheque is for the Membership card replacement. Please make payment within seven (7) working days in order for 
     us to process your application.
iii. Please include a passport size photo for the membership card.

Absent Membership Application Form
You may complete the Absent Membership Application Form and submit together with documents supporting your overseas posting to the Membership department.

Terms and Conditions:
i.   Absent Membership is only applicable to members who are away from Singapore for a consecutive period of 3 months and more.
ii.  Application is to be submitted with documentary proof (original or Certified True Copy) within 1 month after returning to Singapore.
iii. During the period of absence, members' privileges will be suspended. He and his family shall not be entitled to use the facilities 
     of the Club.
iv. Subscription will be refunded to members within 2 months upon receiving the application form.

Application for Concessionary Car Parking Form
Members who wish to enjoy the 50% subsidized parking fees are required to obtain and submit the Application for Concessionary Car Parking Form at the Member Relations Office.
